Hospira I -Secure Syringes

We have been having problems with a couple lots of Hospira's Midazolam 2mg/2ml I-secure syringes. Our nursing staff is complaining that the white plastic plunger does not twist onto the metal screw at the top of the carpuject correctly. They are able to give the drug, but it is an added hassle for a product that is marketed to be more convenient than vials. Has anyone else encountered any issues with them?

We were having a problem also. But, we found out that the nursing staff was not FIRST depressing the rod until it releases from the syringe. They were pulling the rod off of the syringe instead of depressing -- and then the plunger does not screw correctly into the syringe barrel. Have you checked that? It was user error for us.

We had the sales rep do an inservice and I have not had any problems since.
