We use quite a lot, too. Our hospital is about half the size of yours.
After a a number of problems stocking 20,000unit spray kit and spray syringe, everyone agreed to use just the spray syringe with an average of 32/month.
We use some 5000unit vials, only averaging 3/month.
It IS on the high dollar list and these figures are not totally accurate since we've only been Cardinal since May 1.
We use a lot too - about 1200 20mu kits per year. Fortunately the majority of our use is outpatient therefore 340B eligible. Prior to being 340B it was near the top of our list too... now its at the top of our 340B savings report instead!
WE ONLY USE THROMBIN 5000 UNITS BY JP JONES IN OUR O.R. AND 20,000 VERY LITTLE OF NO OTHER STRENGTHS.

We use a ton of Thrombin here at our hospital - with all strengths and forms combined it is usually at or near the top of our 80/20. When I've spoke to different pharms around the country they are surprised to hear that. I do think it has become a habit in our OR and not always needed. It is one of our cost containment projects. I am just wonder - do any of your hospitals use a lot of Thrombin?
